Patient Rights 和 Responsibilities

Every patient has certain 权利responsibilities in regard to their health care. To provide you the very best care, we expect you to be an active 和 informed consumer.

You Have the Right to:

  • Be treated with dignity, 尊重, 和 consideration without regard to your race, 宗教, 年龄, 性, 信仰, or lifestyle
  • Privacy during your clinic visit 和 confidentiality of your health information
  • Interpretation services for deaf 和 hard-of-hearing 
  • Know the names 和 positions of the staff serving you
  • Request to change your provider; request to change your provider from a nurse practitioner to a physician; 和/or request one change of counselor (may require rescheduling)
  • Participate in your health care planning 和 to consent by accepting all or part of treatments offered, or refuse all or part of treatments
  • Information about your diagnosis, treatment, 和 prognosis
  • The opportunity to provide feedback on the services 和 programs offered by the Student Health Center. This input may be offered verbally, in writing, or through the satisfaction survey
  • File a grievance
  • Have access to information about advance care directives.

You Have the Responsibility to:

  • Treat the staff, other patients, 和 visitors with 尊重, consideration, 和 dignity
  • Provide accurate information to the best of your ability about your health, any medications taken (including over-the-counter products 和 dietary supplements), 和 any allergies or sensitivities
  • Inform the staff of any changes in your health status that may affect treatment
  • Ask questions to be sure you underst和 everything regarding your health
  • Report any adverse side effects of treatment 和/or worsening of your condition
  • Follow health care advice 和 medical instructions
  • Arrive at your appointment on time or cancel the appointment 24 hours in advance if unable to keep it
  • If you are unable to care for yourself, you must designate a responsible adult to provide transportation from your appointment, 和 remain with them as indicated by the provider or discharge instructions
  • Accept personal financial responsibility for any charges for the services you receive

Terms to Know

  • Advanced Directive: An Advance Directive is a form that you fill out to describe the kinds of medical care you want to have if something happens to you 和 you can’t speak for yourself. It tells your family 和 your doctor what to do if you’re badly hurt or have a serious illness that keeps you from saying what you want. An advance directive can also be a talk you have with your family 和 your doctor about the kinds of care you want to have. For more information about advance directives, visit:
  • 诊断: the process of determining the nature of a disease or disorder 和 distinguishing it from other possible conditions
  • Discharge Instructions: Information provided to you at the end or after your visit to help a patient underst和 what is known about their condition 和 what was done for them
  • 预后: The likely outcome of an ailment, illness, disease, etc.
